Obtain A Repeat Prescription?

We offer a repeat prescription service to patients who need medication on a regular basis. All requests for repeat prescriptions should be made by using the printed slip from your previous prescription which can be handed in, posted or faxed to the surgery (Fax No. 01527 870507) or by completing the online request form below. We require 48 hours' notice for all prescription requests. If you wish Knights Pharmacy, Catshill to collect your prescription please indicate on the repeat slip. If you wish your prescription to be returned by post, please remember to enclose a stamped addressed envelope.

We regret we do not accept requests for repeat prescriptions over the telephone.
